The Home Matters to Arizona Portfolio of Funds
The Home Matters to Arizona Portfolio of Funds is a centralized application portal that provides applicants access to multiple funding opportunities focused on housing and social determinants of health. Projects submitted through this portal will be evaluated by partner funders based on the components of social determinants of health.

The Portfolio will include funder-specific investment opportunities and facilitate a match with developers who want to access one portal and one application to bring home to life the Home Matters way: strong alignment to SDOH.

Who Should Apply?
The Portfolio of Funds common application is open to all organizations looking to secure funds for a housing-related development project. Applicants do not need to request funding for housing development, but may request funds for the associated physical or program components of the social determinants of health that may support the housing.
This may include items that support education, social inclusion, physical activity, nutrition and healthy food, health care access, social services and behavioral health, open/green space, community leadership, and/or family and child support.
Several funders only fund 501c3 organizations, where others may consider funding for-profit organizations or cities and towns. Applications that include a mix of different kinds of organizations working together are also welcome.
Application will close on April 30, 2025.

Note: If you are looking to apply to the “Housing is Healthcare Fund” (formerly known as “Home Matters Arizona Fund”), please contact Frieda Pollack at fpollack@lisc.org. The “Housing is Healthcare Fund” is not a participating funder in this Portfolio.
